New Patient Information

Is therapy for you?

Do you have questions about therapy and whether it is right for you at this time in your life? Dr. Stokes would be glad to provide a 15-minute free phone consultation to offer some guidance and answer any of your questions. It is a valid question to ask whether therapy is expensive and can I afford it? Yes and yes. Therapy will take time and resources. Anything of value will. You must decide whether the return on the investment in your life and in those you love is worth the therapeutic journey.

New Patient Forms

Before your first appointment, please set up your Patient Portal (Theraportal). Inside the portal, you will complete the intake form, informed consent, and HIPAA privacy notice. Please completed forms prior to your first appointment. If you have questions, please ask! Learning a new system can be confusing and we want to make as simple as possible for you.

We will work with you to make therapy affordable for you. Your insurance is likely to pay the cost of therapy. Currently we accept the following insurance:

Aetna, Avera Health Plan, Cigna Behavioral Health, Humana BH Lifesynch, Iowa Medicaid – (Amerigroup, Iowa Total Care, Molina), Medicare, Multiplan (PHCS / Medishare), Optum, Sanford Health Plan, SD Medicaid, Tricare West, Wellmark

Also, please feel free to ask about sliding scale fees if you do not have insurance.

Your payment can be made out-of-pocket or we can work with your insurer through your employer. Together we will determine if your insurance will cover the cost of therapy.

We will complete a Fee Agreement that outlines how you plan to pay for your therapy services. We currently accept private pay clients and offer a sliding fee scale upon request. Self-pay is typically chosen when someone doesn’t want to use insurance, doesn’t want a recorded diagnosis, or if the insurance carrier is an out-of-network provider. For out-of-pocket payments, payment is due at the end of the month upon receipt of the billing statement.

Dr. Stokes utilizes KASA-Solutions to manage all billing, claims submissions, and HIPAA compliant electronic health records. You will receive a billing statement from New Leaf’s billing agent, KASA. The statement will be from Cedar Rapids, Iowa. Payment is due upon receipt of the bill. You are responsible for your payments and for knowing your own health plan, including coverage and deductibles and co-pays. You are responsible to pay the remainder portion of what your insurance provider doesn’t cover.
